If you are covered by another insurance company, please call our business office at 413-739-5676 to ask about whether we accept your plan.

Please bring your insurance card with you at the time of your visit so we may verify your participation.

Any co-payments required by your plan are due at the time of your appointment.

If your medical bills are to be covered under Worker’s Compensation or auto insurance, please inform our office at the time of your visit. You must provide authorization for your evaluation and treatment.

Patients with HMO/PPO insurance coverage are responsible for obtaining all necessary referrals before visiting our office. If we refer you to a specialist, it is your responsibility to obtain the appropriate referral authorization from our office before seeing the specialist.
