We
are participating providers for a variety of health insurance programs,

Please
bring your insurance card with you at the time of your visit so we
may verify your participation. Co-payments must be paid at the time
of service.
If
your medical bills are to be covered under Worker’s Compensation
or auto insurance please inform our office at the time of your visit.
You must bring with you the authorization for your evaluation and
treatment.
Patients
with HMO/PPO insurance coverage are responsible for obtaining all
necessary referrals before visiting our office. If we refer you to
a specialist, it is your responsibility to obtain the appropriate
referral authorization from our office before seeing the specialist.
